This procedure is often performed when a conventional root treatment or endodontic treatment and re-treatment have failed.

The surgery carries a 85-95% success rate, but is complicated by the multiple canals of the molars, and close proximity to the nerves and sinuses which access to the root more difficult and risky.

Once the tip is removed the tooth becomes less attached to the bony socket, and it can have the effect of making the teeth very slightly mobile. The more the tooth tip is cut back, the more mobile the tooth will be after the operation.

Removing the root tip will also generally weaken the tooth, perhaps leading to a root fracture.
